Health Dept. Okay to open (emailing documentation of water to make snow cone ice) – all
specifications met – earned a perfect score of 100
• Dollar General, 13776 U.S. 69 North, Village Mills – All specifications met – earned
a perfect score of 100
• God’s Green Earth (Farmers Market), 555 Ryan Rd, Sour Lake – all specifications met –
earned a perfect score of 100
• First United Methodist Church Kountze, 1015 S Pine St., Kountze – all specifications met –

• Circle A Convenience Store, 520 Highway 326 South, Sour Lake – Ice machine was closed for
cleaning, food in freezer and in coolers were observed unwrapped, not food specific
labeled or dated- all corrections were made within a 24 hour period, very clean
convenience store – earned a score of 92

for hot and cold hold, cleaning includes: lids on icing containers, grease on floor behind
fryer, hand sinks, some cooking equipment – earned a score of 93

has pans holding up the legs, thawing bags of chicken in a sink of stagnant water, boxes
on the floor in dry storage, cleanliness issues with containers, fans in walk-in coolers and
monitors – earned a score of 88
