during ware washing process - earned a score of 97

score of 100
• Silsbee Quality Meat – 970 East Highway 327, Silsbee - all specifications met – earned a
perfect score of 100
• Love Fellowship – 440 S 12th St., Silsbee - all specifications met – earned a perfect score of

• Village Creek Rehabilitation – 705 N. Main St, Lumberton – water in handwashing sink was
not running with adequate pressure, neither the hand washing sink or the service sink
supplied hot water, cooked food in refrigeration was found dated past 7 days, gnats were
observed swarming near the hand washing sink and in the office area – earned a score of
93

• Kountze Nursing Home - 604 FM 193, Kountze – shelving units holding cooking utensils
and pans have rust build up and are no longer cleanable, under the 3 compartment sink - floor drains, plumbing pipes and under shelving around this area has debris and grime
collected and are attracting gnats, deep cleaning is required, lighting above the steam
table is not working properly and is not giving sufficient lighting, baking sheet pans are
worn and have a flaky build up around all inside edges, wet rags must be placed in
measured sanitizer buckets when not in use or in a designated area for soiled laundry-

men’s restroom, ladies restroom door handle was broken and replaced with a string
wrapped around a dead bolt lock that is too small for the handle hole; paper stuffed

restroom, looked clean and organized – earned a score of 98
• Dairy Queen – 827 N. Main, Lumberton – sanitizer needs to be checked, not working at
correct level, some cleanliness issues on floors, corners, drains and shelves – earned a
